Міщенюк О.Ю. Визначення ролі G1691A алелі гена фактору v коагуляції та G20210A алелі гена протромбіну в розвитку тромбозів у хворих на справжню поліцитемію.
Визначено особливості взаємозв’язку спадкової тромбофілії з розвитком кардіоваскулярних ускладнень у хворих на справжню поліцитемію (СП). Підтверджено, що наявність генетичних маркерів спадкової тромбофілії, зокрема G1691A алелі гена фактору V коагуляції та G20210A алелі гена протромбіну, є тригером виникнення кардіоваскулярних ускладнень у пацієнтів зі СП. Більший внесок генетичних детермінант спадкової тромбофілії в розвиток тромбозів у хворих на СП спостерігається в осіб, які молодші за 60 років, ніж у пацієнтів старшої вікової групи. Високий показник частоти тромбозів у носіїв G1691A алелі гена фактору V та G20210A алелі гена фактору ІІ коагуляції є одним з аргументів на користь доцільності скринінгу хворих на СП на наявність генетичних маркерів спадкової тромбофілії.

Мищенюк О.Ю. Определение роли G1691A аллели гена фактора v коагуляции и G20210A аллели гена протромбина в розвитии тромбозов у больных истинной полицитемии.
Определены особенности взаимосвязи наследственной тромбофилии с развитием кардиоваскулярных осложнений у больных истинной полицитемией (ИП). Подтверждено, что наличие генетических маркеров наследственной тромбофилии, а именно G1691A аллели гена фактора V коагуляции и G20210A аллели гена протромбина, является триггером возникновения кардиоваскулярных осложнений у пациентов с ИП. Более высокий вклад генетических детерминант наследственной тромбофилии в развитие тромбозов у больных ИП отмечается у лиц младше 60 лет, нежели у пациентов старшого возраста. Высокий показатель частоты тромбозов у носителей G1691A аллели гена фактора V и G20210A аллели гена фактора ІІ коагуляции является одним из аргументов в пользу целесообразности скрининга больных ИП на наличие генетических маркеров наследственной тромбофилии.

Mishcheniuk O.Y. The role of the G1691A allele factor v gene and the G20210A allele prothrombin gene in the development of thrombosis in patients with polycythemia vera.
The presence of genetic markers of the inherited thrombophilia, including the G1691A allele of factor V gene and G20210A allele prothrombin gene, is a trigger of cardiovascular complications in PV patients. In PV patients the larger contribution of genetic determinants of the hereditary thrombophilia in the development of thrombosis was observed in individuals younger than 60 years. The high rate of frequency of thrombosis in carriers of the G1691A allele factor V gene and G20210A allele of the factor II gene is one of the arguments in favor of the feasibility of screening patients with PV for the hereditary thrombophilia.

The aim of the study was to determine the incidence of thrombosis in polycythemia vera (PV) patients with genetic markers of the inherited thrombophilia and without. The identifying features of the relationship between the G1691A allele of factor V coagulation gene, the G20210A allele of prothrombin gene and the development of cardiovascular complications in different groups of patients with PV.

Materials and methods. The material for analysis served as the results of clinical and molecular genetic studies 118 PV patients. World Health Organization (WHO) criteria 2008 were used for the diagnosis of PV. Prevalence of thrombosis, patients' age, the presence of cardiac risk factors (CRF) defined by medical records processing. The determination of the G1691A allele of factor V gene and G20210A allele of prothrombin gene was performed with PCR method for amplification of DNA sequence [1]. The difference between the incidence of thrombotic complications per 100 person-year in patients with the presence of genetic markers of the inherited thrombophilia and patients without estimated by the method described by H. Sahai and A. Kurshid [8]. The level of relationship between genetic markers of the inherited thrombophilia and thrombosis was expressed as the rate ratio (RR) with corresponding 95% confidence intervals (CI).

Results. In a total cohort of patients with PV (33.67 per 100 person- year versus 8.24 per 100 person- year, p˂0.0001) and in the younger group of patients without cardiac risk factors (CRF) (48.90 per 100 person- year versus 6.87 per 100 person- year, p˂0.0001) carriers of the G1691A allele factor V gene had higher incidence of thrombosis than patients with the wild-type of allele. It was determined that in the total cohort of PV patients and in group of patients aged less than 60 years without CRF the allelic variant G1691A increased the likelihood of thrombosis in 4,08-fold per 100 person-year (95% CI=3,27-4,88, p˂00001) and 7.11-fold per 100 person-year (95% CI=5,59-8,52, p˂0.0001), respectively. The higher level of the incidence of cardiovascular events was observed among patients with the G20210A allele of prothrombin gene than in carriers of the wild-type gene of coagulation factor II (14.64 per 100 person- year versus 8.31 per 100 person-year, p˂0.0001). The rate ratio of thrombosis, which associated with the presence of G20210A allele prothrombin gene, was 1.76 per 100 person- year (95% CI=1.41-2.12, p˂0.0001). In the group of patients younger than 60 years with CRF carriers of nucleotide variant G20210A coagulation factor II gene had higher incidence of thrombosis (14.92 per 100 person-year versus 5.41 per 100 person-year, p˂0.0001) and high level of RR of cardiovascular events (2.75 times per 100 person-year, 95% CI=1.29-3.71, p˂0.0001), than carriers of the wild-type allele of prothrombin gene.
