Зєльоний І.І. Вплив лікопіду на продукцію прозапальних цитокінів в культурах мононуклеарів периферійної крові хворих на рецидивуючу бешиху.
Обстежено 96 хворих на рецидивуючу бешиху (РБ), з яких 50 хворих (основна група) отримували додатково сучасний імуноактивний препарат лікопід, а 46 пацієнтів (група зіставлення) - тільки загальноприйняту терапію. При проведенні імунологічних досліджень на тлі клінічної маніфестації загострення РБ було встановлено зростання рівня спонтанної продукції прозапальних цитокінів IL-1β та TNFα в культурах мононуклеарів периферійної крові. При загальноприйнятому лікуванні зберігався підвищений рівень спонтаної продукції IL-1β та TNFα на тлі зниження їх стимульованої продукції в культурах мононуклеарів периферійної крові. Включення лікопіду до комплексу лікування хворих на РБ сприяє нормалізації продукції IL-1β та TNFα в культурах мононуклеарів періферійної крові.

Резюме
Зелёный И.И. Влияние ликопида на продукцию провоспалительных цитокинов в культурах мононуклеаров периферической крови больных рецидивирующей рожей.
Обследовано 96 больных рецидивирующей рожей (РР), из которых 50 пациентов (основная группа) получали дополнительно современный иммуноактивный препарат ликопид, а 46 пациентов (группа сопоставления) – только общепринятую терапию. При проведении иммунологических исследований на фоне клинической манифестации обострения РБ было установлено возрастание уровня спонтанной продукции IL-1β и TNFα в культурах мононуклеаров периферической крови. При общепринятом лечении сохранялся повышенный уровень провоспалительных IL-1β и TNFα в крови на фоне снижения их стимулированной продукции в культурах мононуклеаров периферической крови. Применение ликопида в комплексе лечения больных РБ способствует нормализации продукции IL-1β и TNFα в культурах мононуклеаров периферической крови.

Summary
Zelenyi I.I. Influence of likopid on production of cytokines in the cultures mononucleares of periferal blood of the patients with reccurent erysipelas.
The 96 patients with reccurent erysipelas (RE) was investigated. The 50 patients (basic group) got the modern immunoactive preparation likopid and 46 patients (comparison group) - only the generally accepted therapy. During immunological researches on a background clinical manifestation of RE the increase increase the level of spontaneous products of TNFα and IL-1β in the cultures mononucleares of periferal blood was detected. At general accepted treatment took place increase level proinflammatory TNFα and IL-1β at the serum on background decline stimulated production at mononucleares cultures. Aрplication of likopid at the treatment of RE provided to production of TNFα and IL-1β in the cultures of mononucleares.

INFLUENCE OF LIKOPID ON PRODUCTION OF CYTOKINES IN THE CULTURES MONONUCLEARES OF PERIFERAL BLOOD OF THE PATIENTS WITH RECCURENT ERYSIPELAS

We examined 96 patients with recurrent erysipelas, which were divided into two groups – main (50 patients) and control group (46 patients). Treatment of patients in both groups included antibiotics, antiinflammatory, antihistaminic drugs, аscorutinum or ascorbic acid, if necessary, for the purpose of detoxification is also administered saline solutions. In addition, patients of the main group received supplemental immunoactive drug likopid orally  10 mg.

To implement the objectives of the study in both groups studied the level of proinflammatory cytokines – IL-1β and TNFα in the supernatant of  mononuclear cells cultures by enzyme immunoassay for enzyme immunoassay analyzer PR 2100 company «Sanofi Diagnostic Pasteur» (France) using enzyme immunoassay kits produced by «Protein contour»   (St. Petersburg, Russia) - РroConIL-1β and ProConTNFα. Incubation of cell suspension mononucleares cultures of peripheral blood a concentration of cells 1.5 x10⁶/ml medium conducted within one day in CO₂ - incubator at +30 °C. Wherein incubation the cell culture was performed in two ways: without stimulation (for determination of spontaneous cytokine production ) and to PHA stimulation.

Statistical processing of the results of research carried out by statistical methods using a software package StatSoft Statistica 6, taking into consideration the basic principles of statistical methods in clinical drug trials.

When conducting immunological studies in the period of clinical manifestation of recurrent erysipelas found an increase in the spontaneous production of proinflammatory cytokines in mononucleares cultures of peripheral blood: in the main group IL-1β – an average of 6.29 times the norm, TNFα – 2.93 times, in the control group IL-1β – at 6.20 times, TNFα – 2.87 times.

Upon stimulation of cytokine production by using non-specific mitogen (PHA ) in mononucleares cultures found increased production in the main group IL-1β – an average of 2.67 times the norm , TNFα – 2.26 times, in the control group IL-1β – in 2.63 times, TNFα – 2.28 times.

In the main group of patients during treatment with licopid decreased as the spontaneous production and PHA stimulated production of proinflammatory cytokines TNFα and IL-1β in mononucleares cultures blood serum to the upper limit of normal.

In the control group at the time of completion of treatment, spontaneous production of IL-1β standards higher than those on the average 1.63 times (P<0.01), PHA – stimulated production of IL-1β in this period of study was less than 1.25 times the norm (P <0.05), spontaneous production of TNFα exceeded norm  1,33 times, stimulated production – below normal 1.53 times.

Thus, the inclusion licopid in complex treatment of patients with recurrent erysipelas positive effect on the performance of the cytokine profile of blood, helping to optimize their production in mononucleares cultures of peripheral blood of patients. When only the standard treatment of recurrent erysipelas saved significant changes from the production of proinflammatory cytokines mononuclear in peripheral blood of patients. The findings suggest that the inclusion of the modern means of immunoreactive licopid in the complex treatment of patients with acute exacerbation of recurrent erysipelas pathogenesis justified and clinically promising, since the use of this drug helps to normalize the concentration of proinflammatory cytokines (TNFα, IL-1β) in the serum and their products in mononucleares cultures.
