Дяченко М.В., Білько Н.М., Дягіль І.С. Здатність клітин-попередників кісткового мозку пацієнтів із хронічною мієлоїдною лейкемією до автономного росту у суспензійній культурі in vitro без додавання екзогенних ростових факторів.

Визначення факторів, що можуть вказувати на прогресування захворювання при хронічній мієлоїдній лейкемії (ХМЛ) є актуальною темою гематології, онкології та клітинної біології. Оцінка клітин гемопоетичної системи пацієнтів із ХМЛ на різних стадіях диференціювання (стовбурові клітини та клітини-попередники) є тим підходом, який дозволяє виявляти ранні зміни у кровотворенні, що можуть призвести до прогресування ХМЛ. У роботі досліджено такі показники, як здатність гемопоетичних стовбурових клітин та клітин-попередників кісткового мозку підтримувати довготривалу суспензійну культуру без додавання екзогенних ростових факторів,  а також проведено аналіз морфологічних та фенотипових особливостей клітин при культивуванні. Показано, що зміни у функціональних властивостях клітин кісткового мозку є співставними для груп пацієнтів до початку терапії ТКІ та із субоптимальною відповіддю на лікування ТКІ, що може свідчити про накопичення у процесі лікування клітин, які знаходяться у стані спокою та не чутливі до дії ТКІ, а також здатні до секреції аутокринних ростових факторів. Подальше дослідження гемопоетичних клітин пацієнтів із ХМЛ дозволить встановити додаткові механізми, що можуть призводити до прогресування захворювання.

Определение факторов, которые могут указывать на прогрессирование заболевания при хронической миелоидной лейкемии (ХМЛ) является актуальной темой в гематологии, онкологии и клеточной биологии. Оценка состояния клеток гемопоэтической системы пациентов с ХМЛ на разных стадиях дифференцировки (стволовые клетки и клетки-предшественники) является подходом, позволяющим определять ранние изменения в кроветворении, которые могут привести к  прогрессированию ХМЛ. В работе исследованы такие показатели, как способность гемопоэтических стволовых клеток и клеток-предшественников костного мозга к поддержанию длительной суспензионной культуры без добавления экзогенных ростовых факторов, а также проведен анализ морфологических и фенотипических особенностей клеток при культивировании. Показано, что изменения в функциональных характеристиках клеток костного мозга были сопоставимыми для групп пациентов до начала терапи ТКІ и с субоптимальным ответом на лечение ТКІ, что может свидетельствовать о накоплении в процессе лечения клеток, которые находятся в состоянии покоя и не чувствительны к действию ТКІ, а также способны к секреции аутокринных ростовых факторов. Дальнейшее исследование гемопоетических клеток пациентов с ХМЛ позволит определить дополнительные механизмы, которые могут приводить к прогрессированию заболевания.

Identification of factors that may indicate disease progression in chronic myeloid leukemia (CML) is a current issue in hematology, oncology and cell biology. Assessment of hematopoietic cells of CML patients at different stages of differentiation (stem cells and progenitor cells) is an approach that allows determining early changes in the blood, which can lead to the progression of CML. We have studied such factors as the ability of hematopoietic stem cells and bone marrow progenitor cells to maintain a long-term suspension culture without addition of exogenous growth factors, and we have analyzed the morphological and phenotypic features of cells during cultivation. It is shown that changes in the functional characteristics of bone marrow cells were comparable for groups of patients prior to initiating therapy with TKІ and patients with suboptimal response to TKІ treatment that may indicate for accumulation during treatment of cells that are dormant and not sensitive to TKІ and are also capable of autocrine secretion of growth factors. Further investigation of hematopoietic cells of patients with CML will establish additional mechanisms that may lead to the progression of disease.

Ability of bone marrow progenitor cells of patients with chronic myeloid leukemia to autonomous growth in suspension culture in vitro

Chronic myeloid leukemia (CML) is a clonal disorder caused by the acquisition of the genetic rearrangement, Philadelphia (Ph) chromosome (t9;22), by hematopoietic stem cell. Up to date, two major mechanisms have been proposed as an explanation for myeloid cells expansion in chronic myeloid leukemia (CML). One is a reduced susceptibility of hematopoietic stem or progenitor cells to apoptosis, while the other one is an increased activity within the hematopoietic progenitor cell population. Despite the fact that CML was the first hematological malignancy for which the specific treatment was developed, a number of studies indicated that quiescent stem cells in CML are resistant to tyrosine kinase inhibitors (TKI) therapy. That is why, the elaboration of an effective and reliable method for early prognosing of disease progression is crucial for prevention of resistance. Recent evidence indicate that CML myeloid expansion may be caused by altered kinetics at the progenitor, rather than the stem, cell level, therefore assessment of bone marrow progenitor cells functional activity and susceptibility to the external growth factors, could be used for CML status determination. Hence, the aim of this study was to identify the functional, cytogenetic and cytomorphological features of hematopoietic stem and progenitor cells of the bone marrow of patients with CML that are characteristic of disease progression.

Total 44 bone marrow samples were investigated from male and female CML patients with median age of 45,0±8,3 years after an informed consent was obtained. Patients were treated with TKI family drug Imatinib between 6 and 118 month. Each person had undergone hematological and cytogenetic analysis for the Ph chromosome, as well as in vitro investigation of bone marrow mononuclear cells in suspension culture. Obtained samples were divided into two groups according to the patient's therapy response: 1) with optimal response, when no Ph chromosome was detected in the bone marrow and 2) suboptimal response – Ph chromosome was detected in the bone marrow. One more group comprised patients before TKI treatment.

Mononuclear cells were cultivated in concentration of 2x105 cells/ml in DMEM medium (Invitrogen, USA) with 10% FBS (Invitrogen, USA), 50 U/ml penicillin, 50 mg/ml streptomycin (Sigma, USA), 2 mM L-glutamine (Gibco, Germany). Cultures were maintained in 37°С, 5% СО2 and 100% humidity for 21 days. All results were expressed as the mean of quadruplicate cultures. To visualize the morphological characteristics of cells Papenhaim staining method was used. For statistical processing of the results the statistical program Microsoft Excel was used.

Our results have shown that hematopoietic stem and progenitor cells of bone marrow of patients with CML can survive in vitro cultivation with no addition of exogenous growth factors and cytokines at least for the 21 day (for a group of patients before TKI treatment). Hematopoietic cells at different stages of maturation were present in preparations made ​​after 7-day cultivation of bone marrow mononuclear cells in suspension culture. Instead, prevailation of more mature hematopoietic cells was identified after 14 and 21 day of cultivation. The capacity for long-term preservation of viability in suspension culture without addition of exogenous growth factors may indicate the possibility of growth factors autocrine secretion by hematopoietic stem and progenitor cells in CML patients.

The fact the tendency for high cellularity and the prevelance of purly differentiated cells in culture till day 21 was common for cultures of bone marrow mononuclears of patient before TKI treatment and patients with suboptimal response to therapy, suggests the possibility of accumulation of transformed leukemic cells in the bone marrow of patients with progression of disease. These leukemic cells, which are considered to be insensitive to TKI as well as to exogenous growth factors, may afford progression of CML and further development of blast crisis [15]. Further investigation of hematopoietic cells of patients with CML will establish additional mechanisms that may lead to the progression of disease.
