Харківський національний медичний університет МОЗ України Харьковский национальный медицинский университет Kharkiv National Medical University serg_shklyar@ukr.net The purpose of the research consisted in the analysis of correlation intercommunications between organizational and technological factors that can influence on quality of orthopedic constructions. For the study of clinical and technological quality of orthopedic constructions of dentures from the domestic production materials implemented by complex program it was envisaged the studying of the following types of dentures : 20329 artificial crowns (plastic - 13304 units; combined after Bielkin, Borodiuk, Akhmetov - 7025 units), 15621 bridges (plastic - 9789 units; combined - 5832 units), 23538 laminar dentures (partial - 11340 units; complete - 12190 units) and 8903 bugel prostheses (with fixative elements as clammers of the Nei's system - 7100 units and with lock fixation - 1803 units). The total amount of the analyzed constructions of dentures is 68391 units; the total amount of dentists-orthopedists is 75 persons, dental technicians - 103 persons. In the early period of clinical exploitation the determinant factors of constructions quality are a structure of dental clinics peopleware with doctors, in particular an index of peopleware (F9: КС=0,727) and a specific weight of dentists-orthopedists of the first qualification category (F2: КС=0,453), while in a remote the most influential factor is qualification of dental technicians: specific weight of dental technicians without a qualification category (F8: КС=0,517). For providing of crowns quality in the early period the most meaningful is the level of provision of MPI with dentists-orthopedists of the second (and higher) qualification categories (F3: rXY =-0, 325) and dental technicians of the first qualification category, and in a remote it is revealed the reverse middle force intercommunication with specific weight of dental technicians without a category (F6: rXY =-0, 402). The most meaningful for bridges in the early period of its clinical exploitation is influence of qualification of dentists-orthopedists, in particular F2 (rXY =-0, 918), F3 (rXY =+0,665), F4(rXY =+0,435). A little bit another regularities are revealed for removable laminar dentures, namely: F2 (rXY =-0, 378), F4 (rXY =-0, 513), F6 (rXY =-0, 256). It should be noted that quality of bugel prostheses in a greater degree is determined by qualification of dental technicians F7 (rXY =-0, 400), F8 (rXY = +0,482) than by qualification of dentists-orthopedists F2 (rXY =-0, 509). As it is shown on the correlogram of intercommunications of technological quality index (pic. 2) in the generalized form it is possible to come to the conclusion that the most meaningful factor of quality of dentures orthopedic constructions making are a structure of medical staff and its qualification. Thus, regardless of the indicated factors clinical exploitation of orthopedic constructions is marked by the decrease of their quality that cannot be explained by the medical staff qualification and it needs more detailed study from the positions of dental material science and technology of dentures making. Conclusions. 1. Quality of artificial crowns in the early period of clinical exploitation depends on the level of provision of dentists-orthopedists of the second (and higher) qualification categories (F3: rXY =-0, 325) and dental technical laboratories - of dental technicians of the I qualification category; in the remote period it is revealed the reverse middle force intercommunication with specific weight of dental technicians without a category (F6:rXY =-0, 402) and quality of orthopedic construction. 2. The most meaningful for bridges in the early period of their clinical exploitation appeared to be the influence of qualification of dentists-orthopedists, in particular F2 (rXY =-0, 918), F3 (rXY = +0,665), F4 (rXY = +0,435). 3. Both in the early and in the remote periods of clinical exploitation quality of laminar dentures depends on the staff qualification, denture construction and technical equipment of dental technical laboratory: F2 (rXY =-0, 378), F4 (rXY =-0, 513), F6 (rXY =-0, 256). 4. The quality of bugel prostheses in a greater degree is determined by qualification of dental technicians F7 (rXY =-0, 400), F8 (rXY = +0,482) than by qualification of dentists-orthopedists F2 (rXY =-0, 509). The prospects of further researches of intercommunications between quality of orthopedic constructions in the early and remote periods of their clinical exploitation are related to the influence estimation of properties of dental materials, clinical and technological factors of treatment.
